NOTICE TO RETIREES AND THEIR DEPENDENTS
We are very pleased to inform you that the Defense Manpower Data Center (DMDC) will provide verification, re-verification and ID card-issuing services for you and your family members from 12 to 30 September 2005. It is very important that you take advantage of these services. Even if you have an ID card that has not yet expired or is indefinite, it is essential that you come to this location. We need to verify your current status and address and be sure that all changes in family status (marriage, birth or adoption of a child) have been captured in our database. This will help us to ensure that you receive important information from the various agencies of the Department of Defense and that your claims for health services under TRICARE are not denied.
All appointments during this visit will be conducted at the Manila Diamond Hotel , across from the U.S. Embassy ( Roxas BLVD corner Dr. J. Quintos Street ) manila, Philippines . Appointment letters are being sent our however, if you do not get a letter you may show up anyway and get a new ID card for yourself and/or your spouse and children.
From the Director of RAO Subic Bay: For those of you who have been denied an ID card for your spouse and children it is very important for you to go there and bring all your supporting documents and get processed.
SUPPORTING DOCUMENTS:
- Current or expired ID card
- DD-214 (original)
- Marriage contract (original)
- Divorce decree (original)
- Death certificate of a spouse (original)
- Adoption papers of a child (original)
- For children 18 and enrolled in college, student verification letter.
